Neither is a size — they are separate characters that happen to be drawn small. Small caps come from the phonetic alphabet blocks, where they represent specific speech sounds, and superscript letters come from a block meant for phonetic and mathematical notation. Both are incomplete: small caps has no proper lowercase Q or X, and superscript is missing q, so those letters fall back to the ordinary form and the effect breaks on any word containing them. That is not a fault in the tool; those characters simply do not exist in Unicode, because nobody needed them for phonetics.
